BACKGROUND: The utilization of subjective questionnaires for assessing conservative treatment in knee osteoarthritis may present challenges in identifying differences due to inadequate statistical power. Objective tools, such as three-dimensional (3D) kinematic analysis, are accurate and reproducible methods. However, no high-quality studies assessing the effects of intra-articular viscosupplementation (VS) have been published. Therefore, the objective of the study was to evaluate gait kinematics of patients with advanced knee osteoarthritis after VS. METHODS: Forty-two patients were randomized to receive either VS or saline injection (placebo). They underwent 3D kinematic gait analysis before and at 1, 6, and 12 weeks after treatment and knee angles during stance phase were determined. Patients and the healthcare team responsible for data collection, processing, and analysis were blinded to group allocation. Between-group comparisons were conducted using linear mixed models. RESULTS: Compared with placebo, the VS increased the maximum knee extension (3.2° (0.7-5.7)) and decreased the maximum knee flexion (-3.6° (-6.1 to -1.2)) on the sagittal plane at 1 week. At 6 weeks, the VS group sustained a reduced maximum knee flexion (-2.6° (-5.2 to 0.0)). On the axial plane, the VS group demonstrated an increase in maximum internal rotation at 12 weeks (3.9° (0.3 to 7.7)). The VS group exhibited reduced single-leg stance time at 1 week and increased total stance time at 12 weeks. CONCLUSIONS: VS led to short- and long-term kinematic improvements in the sagittal and axial planes, leading to a gait pattern closer to that observed in individuals with less severe osteoarthritic knees.

Prior studies have explored the relationship between knee valgus and musculoskeletal variables to formulate injury prevention programs, primarily for females. Nonetheless, there is insufficient evidence pertaining to professional male soccer players. Here, the aim was to test the correlation of lateral trunk inclination, hip adduction, hip internal rotation, ankle dorsiflexion range of motion, and hip isometric strength with knee valgus during the single-leg vertical jump test. Twenty-four professional male soccer players performed a single-leg vertical hop test, hip strength assessments, and an ankle dorsiflexion range of motion test. A motion analysis system was employed for kinematic analysis. Maximal isometric hip strength and ankle dorsiflexion range of motion were tested using a handheld dynamometer and a digital inclinometer, respectively. The correlation of peak knee valgus with peak lateral trunk inclination was .43 during the landing phase (P = .04) and with peak hip internal rotation was -.68 (P < .001). For knee valgus angular displacement, only peak lateral trunk inclination presented a moderate positive correlation (r = .40, P = .05). This study showed that trunk and hip kinematics are associated with knee valgus, which could consequently lead to increased knee overload in male professional soccer players following a unilateral vertical landing test.

BACKGROUND: Latissimus dorsi transfer is a surgical procedure that can be used for treating posterosuperior rotator cuff tears. The procedure leads to improved shoulder function via alterations in the force vector couple around the shoulder. However, there is still no consensus on the biomechanical changes resulting from latissimus dorsi transfer. METHODS: We performed a systematic review of the literature on 3D motion analysis studies evaluating the effects of latissimus dorsi transfer on shoulder kinematics. The available data on segment and joint range of motion was extracted and subject to meta-analysis when consistent across the studies. FINDINGS: Our meta-analysis of pre- and post-operative studies revealed a significant improvement in forward flexion and abduction following latissimus dorsi transfer. When comparing the latissimus transferred shoulder with an uninjured contralateral side the meta-analysis found no significant difference in flexion range of motion, while abduction and external rotation was significantly higher in the uninjured shoulders. The overall risk of bias was moderate to high. High heterogeneity was found in the reporting of data, which limited our ability to perform a meta-analysis across the studies for all interest outcomes. INTERPRETATIONS: Our findings suggest that latissimus dorsi transfer for posterosuperior rotator cuff tears effectively improves shoulder flexion and abduction. External rotation is also expected to improve but at inferior levels as compared to the unaffected side. However, the heterogeneity of the reported data on 3D motion analysis studies highlights the need for better standardization in research and reporting as to conclude the impact of different joints.

BACKGROUND: Previous authors have utilized gait kinematics to categorize knee osteoarthritis patients into four distinct profiles: (1) flexed knee; (2) externally rotated knee; (3) stiff knee; and (4) knee varus thrust and rotational rigidity. However, the relationship between these gait profiles and patients' characteristics remains poorly understood. Thus, this study aimed to investigate whether differences in clinical and radiographic characteristics were associated with these four gait profiles. METHODS: This cross-sectional study used available data from a previous biomechanical study. Data on the four gait profiles were collected from 42 patients with advanced knee osteoarthritis. Three-dimensional kinematics of the knee was recorded during gait using an optoelectronic system. Subjects were evaluated for knee strength, range of motion, tibial slope, femorotibial angle, radiographic severity, anthropometric measurements, and patient-reported outcomes. Multiple comparisons were made using Dunn's test. The level of significance was set at 5%, and the effect size was calculated. FINDINGS: Body mass index (BMI) was the only variable associated with a specific gait profile: profile 4 (P = 0.01; effect size = P1 × P4: -0.62; P2 × P4: -0.41; P3 × P4: -0.40). INTERPRETATION: Our findings suggest that most clinical and radiographic characteristics commonly measured in clinical practice did not differ significantly among knee osteoarthritis patients with the four different gait profiles. The only exception was a higher BMI noted in those with gait profile 4; however, it remains unclear whether it can cause varus thrust or rotation rigidity. The incorporation of three-dimensional motion analysis to identify gait profiles provided clinical insights beyond the limitations of traditional clinical assessments.

Two-dimensional video analysis systems (2DVAS) are commonly used by clinicians and researchers to determine angles during running. The aim of this systematic review (PROSPERO: CRD42022322798) was to synthesize the literature on the criterion validity and reliability of 2DVAS for measuring angles during running compared to three-dimensional motion analysis systems (3DMAS). We searched for articles on MEDLINE/Pubmed, EMBASE, SciELO, and LILACS up to October/2022. We included studies that evaluated the validity of 2DVAS (when compared to 3DMAS) and/or the reliability of 2DVAS measurements of lower limb and trunk angles during running. Qualitative and quantitative analyses were performed. Seven hundred and five studies were found and 17 were included. Ten studies analysed criterion validity between 2DVAS and 3DMAS and the results ranged from poor to excellent, with most of the parameters assessed presenting poor or moderate validity. Inter-rater reliability of 2DVAS was assessed in nine studies and most of the parameters investigated had good to excellent reliability. Intra-rater reliability (between-day processing) of angular running parameters - investigated in ten studies - was considered excellent for most of the parameters analysed. Inter-session reliability was assessed in three studies and was defined as good or excellent for most of the variables assessed. 2DVAS is a reliable method for measuring joint angles during running. However, the validity of 2DVAS compared to 3DMAS ranges from low to moderate for most running parameters. Therefore, based on the available evidence, caution should be taken when applying 2DVAS, particularly for frontal and transverse plane angles.

BACKGROUND: Foot pathologies can affect the kinetic chain during gait, leading to altered loading at other joints that can lead to subsequent pathologies. Although hallux valgus is the most common foot disease, little has been discussed about the biokinetic effects of hallux valgus on the foot and lower limb. This systematic review evaluated the kinematic, kinetic, and pedobarographic changes of the hallux valgus foot compared to a healthy one. METHODS: Several electronic databases were searched up to January 2022, including only cross-sectional studies with clearly defined isolated hallux valgus diseases and healthy groups. Two investigators independently rated studies for methodological quality using the NIH Study Quality Assessment Tool for cross-sectional studies. Kinetic data were extracted, including temporal data, kinematics of the foot joint, kinematics of the proximal lower limb, and pedobarography. We did meta-analyses tests with a random effects model using the metafor package in R. RESULTS: Hallux valgus patients walk slower compared to a disease-free control group -0.16 m/s (95% CI -0.27, -0.05). Hallux valgus patients exhibited significantly reduced coronal plane motion of the hindfoot-shank during preswing 1.16 degrees (95% CI 0.31, 2.00). Hallux valgus patients generated less force in the hallux region 33.48 N (95% CI 8.62, 58.35) but similar peak pressures in the hallux compared to controls. Hallux valgus patients generated less peak pressure at the medial and lateral hindfoot as compared to controls: 8.28 kPa (95% CI 2.92, 13.64) and 8.54 kPa (95% CI 3.55, 13.52), respectively. CONCLUSION: Although hallux valgus is a deformity of the forefoot, the kinematic changes due to the pathology are associated with significant changes in the range of motion at other joints, underscoring its importance in the kinetic chain. This is demonstrated again with the changes of peak pressure. Nevertheless, more high-quality studies are still needed to develop a fuller understanding of this pathology.

OBJECTIVE: To determine the effects of different therapeutic interventions that have ever been evaluated in randomized controlled trials on pain due to plantar fasciitis. METHODS: We searched different electronic databases until September 2022. Mean differences (MDs) and 95% confidence intervals (CIs) were calculated. The Grading of Recommendations Assessment, Development and Evaluation was used to evaluate the overall certainty evidence. RESULTS: A total of 236 studies met the study criteria, including 15,401 patients. Botulinum toxin MD -2.14 (CI: -4.15, -0.14), micronized dehydrated human amnion/chorion membrane injection MD -3.31 (CI: -5.54, -1.08), dry needling MD -2.34 (CI: -4.64, -0.04), low-dye taping MD -3.60 (CI: -4.16, -3.03), low-level laser therapy MD -2.09 (CI: -2.28, -1.90), myofascial releases MD -1.79 (CI: -2.63, -0.94), platelet-rich plasma MD -2.40 (CI: -4.16, -0.63), radiofrequency MD -2.47 (CI: -4.65, -0.29), and stretching MD -1.14 (CI: -2.02, -0.26) resulted in being effective treatments for pain when compared to the control in the short term. In the medium and long term, only extracorporeal shock wave therapy MD -0.97 (CI: -1.13, -0.81)/MD -2.49 (CI: -3.17, -1.82) was effective for improving pain when compared to the control. CONCLUSIONS: Considering the available studies, this systematic review and meta-analysis showed that different therapeutic interventions seem to be useful strategies for improving pain in patients with plantar fasciitis. In the medium and long term, only extracorporeal shock wave therapy was effective in improving pain when compared to the control.

ABSTRACT Muscle strength is an essential part of the functional assessment of health professionals to select and analyze the effects of clinical interventions. This study aimed to determine the influence of gender and age on isometric strength of hip and knee muscle groups. A total of 127 subjects (50.4% men), aged from 20 to 49 years (stratified into three groups: 20-29 years; 30-39 years; and 40-49 years) participated in this study. A hand-held dynamometer was used to measure isometric normalized torque of the hip abductors, hip external rotators, knee extensors, and knee flexors muscles. Regressions and a two-way analysis of variance were used to identify the influence of age and gender on torque of each muscle group. Age and gender were included in the regression model for all groups. Generally, men aged 20-29 and 30-39 were stronger than age-paired women. For participants aged 40-49, torque was similar for men and women for all muscle groups. There was no difference among age groups for women. Generally, young men were stronger than older men. The association between age and gender in hip and knee strength was proved and liable of subgroup stratification after measurements with a hand-held isometric dynamometer.


RESUMEN La fuerza muscular es un componente básico de la evaluación funcional de los profesionales de la salud para seleccionar y analizar los efectos de las intervenciones clínicas. El objetivo de este estudio fue determinar la influencia del sexo y de la edad en las mediciones de fuerza isométrica de los grupos musculares de la cadera y la rodilla. En el estudio participaron 127 sujetos (50,4% hombres), de entre 20 y 49 años de edad (estratificados en grupos: 20 a 29 años; 30 a 39 años; y 40 a 49 años). El torque isométrico normalizado de los abductores y rotadores externos de la cadera y de los extensores y flexores de la rodilla se midió con un dinamómetro manual. Se utilizaron regresiones y el análisis de varianza (Anova) para identificar la influencia de la edad y el sexo en el torque. Tanto la edad como el sexo se incluyeron en el modelo para todos los grupos musculares. En general, los hombres de entre 20 y 29 años y los de 30 a 39 mostraron tener más fuerza que las mujeres del mismo grupo de edad. Para los participantes de 40 a 49 años, el torque fue similar entre hombres y mujeres para todos los grupos musculares. No hubo diferencias entre los grupos de edad en el grupo de mujeres. En general, los hombres más jóvenes demostraron ser más fuertes que los hombres de mediana edad. La relación entre la edad y el sexo en la fuerza muscular de la cadera y la rodilla se probó y demostró ser susceptible a la estratificación después de las mediciones realizadas con el dinamómetro manual.


RESUMO A força muscular é um componente essencial da avaliação funcional de profissionais da saúde para selecionar e analisar efeitos de intervenções clínicas. O objetivo do estudo foi determinar a influência do sexo e da idade sobre medidas de força isométrica de grupos musculares do quadril e do joelho. Participaram da pesquisa 127 sujeitos (50,4% homens), com idade de 20 a 49 anos (estratificados em grupos: 20 a 29 anos; 30 a 39 anos; e 40 a 49 anos). O torque isométrico normalizado de abdutores e rotadores externos de quadril e extensores e flexores de joelho foi medido com dinamômetro manual. Regressões e uma análise de variância (Anova) foram usados para identificar a influência da idade e do sexo sobre o torque. Tanto idade quanto sexo foram incluídos no modelo para todos os grupos musculares. Em geral, homens de 20 a 29 anos e de 30 a 39 anos demonstraram mais força do que mulheres da mesma faixa etária. Para participantes de 40 a 49 anos, o torque foi similar entre homens e mulheres para todos os grupos musculares. Não houve diferença entre as faixas etárias no grupo de mulheres. Em geral, homens mais jovens se mostraram mais fortes do que homens mais velhos. A relação entre idade e sexo na força muscular do quadril e do joelho foi provada e se mostrou passível de estratificação após as medições feitas com o dinamômetro manual.

Background: The movement of the arm relative to the trunk results from 3-dimensional (3D) coordinated movements of the glenohumeral (GH) and scapulothoracic (ST) joints and dictates the scapulohumeral rhythm (SHR). Alterations in SHR increase joint overload and may lead to low functional scores, pain, and failures in patients undergoing reverse total shoulder arthroplasty (RSA). The goal of this systematic review and meta-analysis was to examine 3D SHR kinematics after RSA and compare it to that of asymptomatic shoulders. Methods: A systematic review and meta-analysis of articles in English were performed using PubMed, Embase, Cochrane Library, and SciELO. Additional studies were identified by searching bibliographies. Search terms included "Reverse shoulder arthroplasty", "3D", and "scapula". It was selected cross-sectional studies that reported SHR with 3D motion analysis systems in patients who underwent RSA and asymptomatic controls. Two authors independently performed the extraction of articles using predefined data fields, including study quality indicators. Results: Data from four studies were included in quantitative analysis, totaling 48 shoulders with RSA and 63 asymptomatic shoulders. Pooled analyses were based on random-effects model (DerSimonian-Laird). A statistically smaller SHR ratio was observed in the RSA group than that in the control group (P < .00001), meaning a greater contribution of ST joint in relation to GH joint for arm elevation. The standardized mean difference was -1.16 (95% confidence interval: -1.64, -0.67). A sensitivity analysis with three more studies that had imputed data on control group did not change the direction of the effect. The standardized mean difference on sensitivity analysis was -0.60 (P = .03; 95% confidence interval: -1.13, -0.06). It was detected as "not important heterogeneity" within the comparison (I2: 22%). Chi-square was not statistically significant (Chi2: 3.85), and I2 was 22%. Tau2 was not zero (Tau2: 0.05). Sensitivity analysis showed an I2 of 74%, which might represent substantial heterogeneity, Chi-square was not statistically significant (Chi2: 23.01), and Tau2 was not zero (Tau2: 0.37). Conclusion: This study found that RSA shoulders have an increased contribution of ST joint during arm elevation, compared with asymptomatic shoulders. More movement in ST joint in proportion to GH joint increases GH joint contact forces, which could lead to component loosening or other complications. Further studies should address the clinical implications of this kinematic finding.

OBJECTIVE: We performed a systematic review to investigate the effects of low-level laser therapy (LLLT) on pain and disability in patients with plantar fasciitis (PF). METHODS: We searched Pubmed, PEDro database, Scielo, and Cochrane Central for randomized controlled trials that evaluated the effects of LLLT for patients with PF. The methodological aspects of the studies included were scored using the PEDro scale. Three comparisons were made: LLLT compared with placebo, LLLT combined with conventional rehabilitation (CR) compared with CR and LLLT compared with extracorporeal shock wave therapy. RESULTS: Fourteen studies (817 patients) met the study criteria. Compared to the placebo group, LLLT improved pain (MD, -2.3; 95% CI: 2.6 to -2, I2 = 0%; 4 studies, N = 234: moderate-quality evidence) in the short term (0-6 weeks). No significant difference in short-term disability was found for participants in the LLLT group compared to the placebo group. Compared to the CR group, LLLT combined with CR improved pain (MD, -2.0; 95% CI: 2.9 to -1.1, I2 = 0%; 2 studies, N = 90: moderate-quality evidence) in the short term (0-6 weeks). Compared to extracorporeal shock wave therapy, LLLT did not significantly reduce pain intensity in the short term (MD, 0.5; 95% CI: 2.0 to 2.9, I2 = 96%; 4 studies, N = 175: low-quality evidence). CONCLUSIONS: LLLT may improve pain in the short term and can be considered as a component of care of patients with PF. However, this superiority disappeared compared to extracorporeal shock wave therapy. LEVEL OF EVIDENCE: Therapeutic level I.

BACKGROUND: Determine whether knee kinematics features analyzed using machine-learning algorithms can identify different gait profiles in knee OA patients. METHODS: 3D gait kinematic data were recorded from 42 patients (Kellgren-Lawrence stages III and IV) walking barefoot at individual maximal gait speed (0.98 ± 0.34 m/s). Principal component analysis, self-organizing maps, and k-means were applied to the data to identify the most relevant and discriminative knee kinematic features and to identify gait profiles. FINDINGS: Four different gait profiles were identified and clinically characterized as type 1: gait with the knee in excessive varus and flexion (n = 6, 14%, increased knee adduction and increased maximum and minimum knee flexion, p < 0.01); type 2: gait with knee external rotation, either in varus or valgus (n = 11, 26%, excessive maximum and minimum external rotation, p < 0.001); type 3: gait with a stiff knee (n = 17, 40%, decreased knee flexion range of motion, p < 0.001); and type 4: gait with knee varus 'thrust' and decreased rotation (n = 8, 19%, increased and reduced range of motion in the coronal and transverse plane, respectively, p < 0.05). INTERPRETATION: In a group of patients with homogeneous Kellgren-Lawrence classification of knee OA, gait kinematics data permitted to identify four different gait profiles. These gait profiles can be a valuable tool for helping surgical decisions and treatment. To allow generalization, further studies should be carried with a larger and heterogeneous population.

ABSTRACT BACKGROUND AND OBJECTIVES: This study aimed to identify the association between hip muscle strength and the scores from subjective functional and psychological evaluation questionnaires in patients with chronic hip pain. METHODS: Fifty-fivepatients with painful hip injuries (30 males) performed isometric peak strength tests of the abductors, extensors, and internal and external rotators of the hips with a hand-held dynamometer. The degree of functionality was measured by the Hip Outcome Score (HOS) and Lower Extremity Functional Score (LEFS), pain was estimated by the Visual Analog Scale (VAS) and kinesiophobia was calculated using the Tampa questionnaire. The Pearson correlation coefficient was used (alfa=5%) to test the associations between the muscle strength and the scores from the questionnaires. RESULTS: There were significant correlations between the strength of all four hip muscles and the HOS (r>0.29). Only the hip external rotators showed a significant correlation with pain (r=-0.30). No significant correlations were found for LEFS (r<0.24) and Tampa questionnaires (r¬0.15). CONCLUSION: The reduction in peak strength of the hip extensors, abductors and external rotators was associated with a reduction in the level of hip functionality but did not correlate with neither the level of overall functionality of the lower limbs nor the degree of kinesiophobia. Also, a reduction of hip external rotators strength was related to an increase in the intensity of pain.


RESUMO JUSTIFICATIVA E OBJETIVOS: Este estudo teve como objetivo identificar a associação entre a força muscular do quadril e os escores de questionários subjetivos de avaliação funcional e psicológica em pacientes com dor crônica no quadril. MÉTODOS: Cinquenta e cinco pacientes com lesões dolorosas no quadril (30 homens) realizaram testes isométricos do pico de força de abdutores, extensores e rotadores internos e externos do quadril com um dinamômetro portátil. O grau de funcionalidade foi medido pelo Hip Outcome Score (HOS) e Lower Extremity Functional Score (LEFS), a dor foi avaliada pela escala analógica visual (EAV) e a cinesiofobia foi calculada pelo questionário de Tampa. O coeficiente de correlação de Pearson foi utilizado (alfa=5%) para testar as associações entre a força muscular e os escores dos questionários. RESULTADOS: Houve correlações significativas entre a força de todos os quatro músculos do quadril e o HOS (r>0,29). Apenas os rotadores externos do quadril apresentaram correlação significativa com a intensidade da dor (r=-0,30). Nenhuma correlação significativa foi encontrada para LEFS (r<0,24) e questionário de Tampa (r¬0,15). CONCLUSÃO: A redução no pico de força dos extensores, abdutores e rotadores externos do quadril foi associada à redução no nível de funcionalidade do quadril, mas não se correlacionou com o nível de funcionalidade geral dos membros inferiores ou com o grau de cinesiofobia. Além disso, uma redução da força dos rotadores externos do quadril foi relacionada a aumento na intensidade da dor.

Background: The acetabular labrum plays a major role in hip function and stability. The gold standard treatment for labral tears is labral repair, but in cases where tissue is not amenable to repair, reconstruction has been demonstrated to provide superior outcomes compared to debridement. Many types of grafts have been used for reconstruction with good to excellent outcomes. Autograft options include iliotibial band (ITB), semitendinosus, and indirect head of the rectus femoris tendon, while allografts have included fascia lata and gracilis tendon allografts. Questions/Purposes: As allografts are not always readily available and have some inherent disadvantages, the aims of this systematic review were to assess (1) indications for labral reconstruction and (2) summarize outcomes, complications, and reoperation rates after arthroscopic labral reconstruction with autografts. Methods: A systematic review of the literature was performed using six databases (PubMed, CINAHL, Cochrane Central Register of Controlled Trials, Cochrane Database of Systematic Reviews, Scopus, and Google Scholar) to identify studies reporting outcomes for arthroscopic labral reconstruction utilizing autografts, with a minimum follow-up of 1 year. Study design, patient demographics, autograft choice, complications, donor site morbidity, reoperation rates, conversion to arthroplasty, and patient reported outcomes were extracted and reported. Results: Seven studies were identified for inclusion with a total of 402 patients (173 females, age range 16-72, follow-up range 12-120 months). The most commonly reported functional outcome score was the modified Harris Hip Score (mHHS), which was reported in six of seven studies. Preoperative mHHS ranged from 56 to 67.3 and improved postoperatively to a range of 81.4-97.8. Conversion to total hip arthroplasty and reoperation rates ranged from 0 to 13.2% and 0 to 11%, respectively. The most common indication for labral reconstruction was an irreparable labrum. Autografts utilized included ITB, hamstring tendons, indirect head of rectus femoris, and capsular tissue. Conclusions: Arthroscopic autograft reconstruction of the acetabular labrum results in significant improvement in the short- and mid-term patient reported outcomes, for properly selected patients presenting with pain and functional limitation in the hip due to an irreparable labral injury.

BACKGOUND: Dynamic valgus has been the focus of many studies to identify its association to an increased risk of running-related injuries. However, it is not known which physical and biomechanical variables are associated with this movement dysfunction. RESEARCH QUESTION: This study aimed to test the correlation between strength, flexibility and biomechanical variables and dynamic valgus in female runners. METHODS: Twenty-nine healthy females ran on a treadmill at 2.92 m/s and performed strength, range of motion and endurance tests. Pelvic, hip and ankle kinematics were measured with a 3D motion analysis system. Six multiple linear regression models were used to identify the ability of physical and biomechanical variables to predict excursion and peak of contralateral pelvic drop, hip adduction and internal rotation. RESULTS: Contralateral pelvic drop and hip adduction were positively correlated to ankle eversion and step cadence. Hip internal rotation had a negative correlation with ankle eversion. Despite significance, predictor variables explained less than 30% of dynamic valgus variance during running. No interest variable had significant correlation with the hip strength and hip and ankle passive range of motion. SIGNIFICANCE: The results showed that distal joint kinematics and spatiotemporal variables should be considered during biomechanical running analysis to identify their possible relationship with joint overload caused by dynamic valgus. Caution should be taken when linking hip disorders during running to posterolateral hip strength and stiffness, core endurance, and ankle dorsiflexion range of motion since no correlation occurred amongstthese variables in this sample of female runners.

The aim of this study was to test the correlation between knee-to-hip flexion ratio during a single leg landing task and hip and knee strength, and ankle range of motion. Twenty-four male participants from a professional soccer team performed a continuous single leg jump-landing test during 10s, while lower limb kinematics data were collected using a motion analysis system. After biomechanical testing, maximal isometric hip (abduction, extension, external rotation), knee extension and flexion strength were measured. Maximum ankle dorsiflexion range of motion was assessed statically using the weight bearing lunge test. Pearson correlation coefficients were calculated to determine the associations between the predictor variables (knee and hip strength, and ankle ROM) and the main outcome measure (knee-to-hip flexion ratio). Correlation between knee-to-hip flexion ratio and hip abductors strength was significant (r = -0.47; p = 0.019). No other significant correlations were observed among the variables (p > 0.05). These results demonstrated that a lower hip abductors strength in male soccer players was correlated with a high knee-to-hip flexion ratio during landing from a single leg jump, potentially increasing knee overload by decreasing energy absorption at the hip. The results provide a novel proposal for the functioning of hip muscles to control knee overload.

Objetivo: O objetivo deste estudo foi comparar a co-contração muscular (CCM) e ativação dos músculos vasto lateral (VL) vasto medial oblíquo (VMB) durante a marcha e uma amostra saudável ­ grupo controle (GC) e pacientes submetidos a reconstrução do ligamento cruzado anterior (RLCA). Métodos: Vinte e três indivíduos participaram neste estudo, 14 GC e 9 RLCA. A atividade mioelétrica do VL e VMO foram captados para cálculo da CCM. A razão VL/VMO foi obtida dividindo o sinal normalizado desses dois músculos em cada ponto da curva. O valor da CCM e a relação de ativação na fase de apoio duplo, fase de apoio simples, fase de apoio terminal e fase de balanço foram obtidas pelo cálculo da média aritmética dos valores de intensidade da curva comum em cada intervalo. Resultado: CCM foi significativamente menor no grupo RLCA durante a fase de apoio dupla (p=0.001), efeito máximo (1.72). Não foram encontradas diferenças entre as outras comparações. Conclusão: O resultado desse estudo mostrou que a contração dos músculos VL e o VMO na fase inicial de apoio duplo da marcha foi diferente entre indivíduos saudáveis e submetidos a RLCA. Este achado pode estar relacionado a diminuição da estabilidade patelofemoral durante a resposta a carga, aumentando o potencial risco de desenvolver lesões nesta articulação.


Objective: The aim of this study was to compare vastus lateralis and vastus medialis oblique (VL/VMO) muscle co-contraction (MCC) and activation ratio during gait between healthy subjects- control group (CG), and those with anterior cruciate ligament reconstruction (ACLR). Methods: Twenty-three subjects participated in this study, 14 CG and 9 ACLR. The myoelectric activities of the VL and VMO were captured to calculate the MCC. The VL/VMO ratio was obtained by dividing the normalized signals of these two muscles at each point of the curve. The MCC values and the activation ratio in the initial double limb stance, single limb stance, terminal double limb stance and swing were obtained by calculating the arithmetic mean of the intensity values ​​of the common curve in each interval. Results: MCC was significantly lower in the ACLR group during the initial double limb stance phase (p=0.001), with a high effect size (1.72). No significant differences were found for the other comparisons. Conclusions: The results of this study showed that the VL and VMO muscles co-contraction in the initial double limb stance phase of gait was different between the healthy and ACLR individuals. This finding may be related to lower patellofemoral stability during the loading response, increasing the potential risk for the development of injuries in this joint.

BACKGOUND: Leg length discrepancy (LLD) can be related to different pathologies, due to an inadequate distribution of mechanical loads, as well as gait kinematics asymmetries resulted from LLD. RESEARCH QUESTION: To validate a model to predict anatomical LLD (ALLD) based on gait kinematics. METHODS: Gait of 39 participants with different lower limb pathologies and mild discrepancy were collected. Pelvic, hip, knee and ankle kinematics were measured with a 3D motion analysis system and ALLD, femur discrepancy (FD) and tibia discrepancy (TD) were measured by a computerized digital radiograph. Three multiple linear regression models were used to identify the ability of kinematic variables to predict ALLD (model 1), FD (model 2) and TD (model 3). RESULTS: Difference between peak knee and hip flexion of the long and short lower limb was selected by models 1 (p < 0.001) and 2 (p < 0.001). Hip adduction was selected as a predictor only by model 1 (p = 0.05). Peak pelvic obliquity and ankle dorsiflexion were not selected by any model and model 3 did not retain any dependent variable (p > 0.05). Regression models predicted mild ALLD with moderate accuracy based on hip and knee kinematics during gait, but not ankle strategies. Excessive hip flexion of the longer limb possibly occurs to reduce the limb to equalize the LLD, and discrepancies of the femur and tibia affects gait cycle in a different way. SIGNIFICANCE: This study showed that kinematic variables during gait could be used as a screening tool to identify patients with ALLD, reducing unnecessary x-ray exposure and assisting rehabilitation programs.

The purpose of this study was to test the validity of dynamic leg length discrepancy (DLLD) during gait as a radiation-free screening method for measuring anatomic leg length discrepancy (ALLD). Thirty-three subjects with mild leg length discrepancy walked along a walkway and the dynamic leg length discrepancy (DLLD) was calculated using a motion analysis system. Pearson correlation and paired Student t-tests were applied to calculate the correlation and compare the differences between DLLD and ALLD (α = 0.05). The results of our study showed DLLD is not a valid method to predict ALLD in subjects with mild limb discrepancy.

In recent years, a more precise understanding of the anatomy and biomechanical impair of the knee joint in these cases has enabled great advances in therapeutic approaches. Well-documented studies have shown that the repair of these lesions presents superior functional and clinical improvement when compared with meniscectomy. However, the progression of degenerative joint changes in the long-term still exhibits conflicting results.


A variabilidade da sintomatologia e o fato de não serem facilmente reconhecidas nos exames de imagem tornam o diagnóstico e o tratamento das lesões das raízes posteriores dos meniscos tarefas desafiadoras para o ortopedista. Nos últimos anos, uma compreensão mais precisa da anatomia e do comprometimento biomecânico da articulação do joelho nessas lesões têm possibilitado grandes avanços nas abordagens terapêuticas. Estudos bem documentados demonstram que o reparo dessas lesões oferece uma melhoria clínica e funcional superior à meniscectomia. Entretanto, os resultados da progressão das alterações degenerativas articulares em longo prazo ainda são conflitantes.
